We are seeking a Social Worker to join our team at our assisted living facility You will work alongside other professionals to provide exceptional care to our residents.
We are currently looking for a full:time Social Worker at our Hutchinson location. Please join our team as we serve our residents with a commitment to compassion, excellence, and innovation.
Job Summary
This is a dual role position that oversees the Social Services Department and Activities Department within the building. The social services director is responsible to assist with providing information on services and resources available to help residents cope with their illness. The social worker will assist with coordinating mental health services, providing updates and concerns to mental health practitioners. The social services director is responsible to assist with behavior monitoring and management along with assisting in staff training.
This position also plans, schedules and coordinates the activities program that is suited to the needs, capacities and interests of the residents. The activities program will allow and encourage the resident to function at their highest level. As the facility continues to grow, the activities part of the position will decrease to focus on the Social Worker role permanently.
Essential Duties and responsibilities
Social Work
:Assist other department supervisors when necessary.
:Maintain Social Service documentation
:Complete Admission assessment to be completed within 2 weeks of admission
:Care plans
:Admission notes
:Vulnerable Adult Assessment
:Complete Quarterly, change of condition, hospital re:admission assessments.
:Care plan and Vulnerable Adult assessment update
:Alert care team If necessary (i.e. psychiatrist, case managers, family)
:Complete mood/behavior progress notes quarterly and upon change of condition
:Identify and seek ways to support residents individual needs and preferences, customary routines, concerns and choices.
:Assist with Coordination of mental health provider appointments and communication
:Complete required
paperwork/correspondence for appointment related to residents mental health.
:Assist with coordination and communication with residents mental health care providers.
:Act as liaison between River Oaks at Lake Pepin and the community.
:Complete necessary correspondence with organizations and individuals in the community
:Meetings and Education
:Attend department head meetings and in:service education provided by facility
:Maintain licensure
:Services and procedures
:Assist with intervention for behavior, including observation and monitoring
:Appropriate reporting of changes in client status.
:Ability to either answer questions or direct questions and/or problems outside of office to appropriate supervisor
:Maintain communication with residents guardian, family and other providers regarding resident changes, concerns and conditions, ect
:Organize and coordinate care conference meetings on a quarterly/annual basis. (Resident preference)
:Assist in making outside referrals as needed, recommended or requested.
:Including but not limited to: Mental health providers, ILS, AHRMS services, working, volunteering.
:Provide presentations n Bill of Rights and Vulnerable Adult Act for in:services.
:Writer or update policy/procedures prn as it pertains to Social Services
:Assist with marketing and referral process as needed
:Provide 1:1 visits with residents as needed
:Assist in training other staff with problem solving and behavioral intervention strategies.
Activities Director
:Assist other department supervisors when necessary.
:Maintain activity department records.
:Complete activity interest survey within 2 weeks of admission.
:Complete activity assessment within 30 days of admission, annual thereafter. Significant changes will be documented as they occur.
