Primary tasks:
We offer a dynamic, positive, one-of-a-kind environment that actively promotes the health and well-being of team members and persons served! Due to program expansion We are pursuing Health Care Coordinators to join our team. Sign on bonus and great benefits!! Possible remote home office option. Both full-time and part-time options.

tasks/responsibilities include but are not limited to:
- Providing care coordination to adults with disabilities
- Coordinating approved medical and social services
- Assessing medical and social needs of members ensuring they receive necessary care and required services
- Conducting assessments
- Creating care plans
- Communicating with outside professionals
- Assisting members with care transitions
- Member advocating
- Completing required documentation
- Adhering to timelines
- Supporting members with a spectrum of disabilities
- Traveling through Twin Cities Metro counties conducting community and home meetings
- Proactively contributing to company culture and teams health and well-being
practice:
suitable education
- Required MN RN or LSW license; OR a four-year degree in a closely related field with three or more years of practice in home and community located services, and practice working with people with disabilities.

- Computer expertise - Microsoft Office

- Maintain confidentiality
- capability to take initiative; work independently with minimal supervision

- capability to multi-task, prioritize and work well under multiple demands
- Organized and attentive to detail
- Must have a valid driver’s license and a reliable, insured vehicle with a clean driving record
- Must pass background study

Additional Info:

Benefits located on hours worked include:
- $500 sign on bonus
- Health, Dental, Life, 403B, Holidays and PTO
- Supervision for LSW license provided through agency
